What is Dry Needling and can this help you?

Dry Needling is a targeted therapeutic technique used at PhysioFit Berwick to rapidly relieve muscle pain, eliminate stiffness, and accelerate injury recovery. By inserting thin, sterile, solid filament (Acupuncture) needles into myofascial trigger points (painful muscle knots), our practitioners deactivate hypersensitive tissues to help restore normal, pain-free movement.

Unlike traditional acupuncture, which focuses on Eastern medicine energy pathways, dry needling is strictly rooted in Western medicine, neuroanatomy, and modern musculoskeletal science.

The Science Behind Dry Needling

When a muscle is overused, stressed, or injured, its microscopic fibres contract continuously to form a painful knot. This spasm cuts off local blood supply, starving the tissue of oxygen and locking it in a cycle of chronic pain. Inserting a microscopic needle directly into the knot triggers an immediate healing cascade:

  • Mechanical Release: The needle physically breaks up locked muscle fibres, forcing the tight band to relax.
  • The Local Twitch Response (LTR): The muscle responds with a brief, involuntary twitch. This reflex signals your spinal cord to instantly lower the muscle’s tension.
  • Chemical Normalisation: Inflammatory chemicals and pain neurotransmitters drop significantly immediately after the twitch.
  • Restored Blood Flow: As the knot relaxes, blood vessels reopen. Fresh, oxygenated blood floods the area to repair tissue and flush out metabolic waste.

  • The Insertion: Most patients feel very little as the ultra-thin needle is introduced through the skin.
  • The Twitch Response: When the needle hits the target knot, you may feel a brief, involuntary muscle cramp or “local twitch response.” This is a positive sign that the muscle is resetting.
  • The Duration: Needles are safely left in place for anywhere from 30 seconds up to 20 minutes depending on your condition.
  • Post-Session Soreness: You might experience a mild, dull ache in the treated muscle for 12 to 24 hours, like the feeling after an intense gym workout.

Essential Safety & Precautions

Dry needling is an exceptionally safe, evidence-based treatment when performed by trained professionals. However, please notify your physiotherapist prior to your session if you:

  • Are in your first trimester of pregnancy
  • Take blood-thinning medications (e.g., Warfarin, Aspirin)
  • Have a severe needle phobia
  • Have a compromised immune system or active skin infections

Essential Aftercare Instructions

  1. Keep Moving: Perform gentle, active movements and light stretches as prescribed by your physiotherapist.
  2. Stay Hydrated: Drink plenty of water throughout the day to flush out local tissue metabolic waste.
  3. Apply Heat: If the area feels bruised or sore, a warm bath or heat pack can provide excellent relief.
  4. Modify Activity: Avoid heavy weightlifting or high-intensity sport for 24 hours post-treatment.

Skiing and snowboard injury prevention and treatment

The snow season is well and truly here again with the resorts in full swing…

The majority of skiing injuries occur traumatically, often the result of a fall or collision with another skier or object, such as a tree.

Skiing injuries commonly involve the knee, shoulder, wrist, thumb, head and neck. 

These injuries often include ACL and MCL ligament sprains/tears, medial meniscal tears, quadriceps and groin muscle strains, thumb joint sprains and shoulder dislocations, fractures and bruising, and often concussion, with some being quite serious requiring surgical management.

If you do receive an injury, the best advice is to get it treated as soon as possible.

Prior to treatment – initial RICE management is suggested (Rest, Ice, Compression, Elevation). Avoid alcohol, and no APRE ski.

Avoid Skiing Injuries!

Physiotherapist have been treating ski injuries for years and have been able to produce an accurate account of the “how, when, who, why and where” of injuries.

Critical periods for injury:    

  • First day on the slopes – after a long drive (3.5-6hrs for most of us) all skiers are eager to get straight on the slopes without warming up.’
  • First two hours on the slope – snow is harder, body not warmed up, skiing is a little rusty and body may be a little sore from previous day
  • Just before a break – the body is fatigued from a demanding session
  • Just after a break – the body has had time to cool down and may be out of rhythm. Fatigue is also still a factor
  • Last hour before finishing – the body is well fatigued by now and hunger/thirst begins to take over. Skiing conditions can also become a little harder late in the day.

Quick tips to help avoid injury:

  • Get fit to ski– prepare ahead for the ski season with a tailored program prepared by your physiotherapist to target muscle groups and balance
  • Warm up to increase your body temperature
  • Stretch main muscle groups and practise appropriate skiing movement before heading to the lift queue
  • Do some gentle stretching after a long session on the mountain
  • Consider a practice fall, especially if you have had an injury
  • Check that all your gear is functional/safe, or have it checked for you
  • Snow-boarders should use gloves with wrist guards to avoid wrist fractures during a fall.
